Anchor Clinic New Patients

To make a new patient appointment with our Pensacola clinic please contact 850-433-1656 and our administrative staff will assist you in the process. To make an appointment with a clinician in Destin call 850-650-0555. Please inquire with the staff with any questions regarding insurance information.

Feel free to download our new patient packet to better serve you on your first appointment, however it is not required for you to do so. We will provide you one upon arrival to the office. If you choose to receive your packet upon arrival, please come 15-30 minutes early.

If you will be seeing Dr. Boilini or Dr. Truong at our Destin clinic for a psychiatry appointment, please also download this packet. You will need both sets of forms for your first visit.
